How Active Membership Works
How to Join
Step 1: Enroll
Complete the Founding 25 enrollment form and submit the one-time $99 enrollment fee.
Step 2: Complete Your Health Requirements
Before receiving clinical services, patients must complete all required health history forms, consents, and medical clearance requirements. A Good Faith Exam may be required before treatment.
Step 3: Schedule Your Service
Once eligible, schedule your mobile wellness appointment through the Apex booking system.
Step 4: Enjoy Your Benefits
Your eligible Founding Member benefits will be applied according to the current Founding 25 program terms.
- To keep your Founding Member status active, complete at least one eligible Apex Wellness Co. service every six months.
If no eligible service is completed within a six‑month period, your membership may be considered inactive and Founding Member benefits may end.
Membership benefits are personal to you and cannot be transferred, shared, sold, or gifted to another person.
What Your Membership Does Not Include
The Founding 25 is a preferred-benefits program. It does not include a monthly IV therapy, injection, treatment, or service credit. Services are purchased separately at the applicable Founding Member rate.
The discount does not apply to:
• The Founding 25 enrollment fee
• Other memberships
• Gift cards
• Deposits
• Travel or event coordination fees
• Third-party medical services
• Products or services specifically excluded by Apex Wellness Co.
Important Membership Information
Founding Membership benefits are subject to appointment availability, clinical appropriateness, medical clearance, service-area availability, and Apex Wellness Co. policies.
Membership does not guarantee treatment. Every clinical service requires an individual assessment, and treatment may be modified, postponed, or declined when medically appropriate.
Apex Wellness Co. may update program procedures, exclusions, or administrative policies when necessary. The core 10% Founding Member discount will remain available while the membership is active, subject to the program terms.
Membership fees are nonrefundable unless otherwise required by law.

Does the membership include a free IV?
No. The Founding 25 requires a one-time $99 enrollment fee. There is no recurring monthly membership payment.

The 10% discount may be combined with promotional discounts unless the promotion specifically states otherwise.
Can I apply the discount to another membership?
No. The Founding Member discount cannot be applied toward another membership, gift cards, enrollment fees, or excluded products and services.
Are there really only 25 memberships?
Yes. Enrollment is limited to the first 25 approved members. Once all memberships are claimed, Founding 25 enrollment will close.
