Family Add-On Benefits
Each Family Add-On Member must:

Be enrolled under an active primary membership.

Reside in the same household as the primary member.

Complete all required registration forms, health questionnaires, Good Faith Exam requirements, and consent documents.

Meet all medical eligibility requirements before receiving treatment.
* A maximum of four (4) Family Add-On Members may be enrolled under one primary membership.
* To qualify for Family Add-On pricing, household members should be scheduled at the same location during the same appointment whenever clinically appropriate.
* Separate appointments or different service locations may be subject to standard member pricing or additional travel fees
